If you have something such as black fog on and you flank someone, will the the black fog go away (will you not be hidden anymore?)

Yes fog will be removed, IF the attack goes through and/or the enemy being flanked has First Strike 3.

That's right, if you depend on any Hide to get that x2 weapon damage or in cases of defense ( like needing time to heal or shield ), Flanking is most definitely not a good thing - try to avoid gear that grants this as well. Cheap Shot should be avoided as well.

As long as you do not flank the opponent while hidden, the epic is fine to learn.

If you want an extra attack, then I recommend training Relentless.
"As long as you do not flank the opponent while hidden, the epic is fine to learn."
Battle conditions often throw a flank situation at you whether you've been avoiding it or not; ex. pets going where they shouldn't, doubloons summons and the game AI can sometimes make your "best laid plans gang a gley"*
* Robert Burns - "The best laid plans of Mice and Men aft gang a-gley." Means that something will go wrong.

Flanking can be a pain when combined with hidden, but it is still useful as an epic. The trick is just to be very observant when you move your character, and also to position your companions so that they won't trigger it. That being said, if you want to keep it simple, yeah don't bother with flanking. It's one of those things that takes extra work, but can yield some nice rewards.
